Hilary is a Board Member of the West Coast Chapter of the Private Equity CFO Association. She is a Certified Public Accountant and holds a Bachelor of Science in Accounting from Pepperdine University.

Michael R. Kramer was Senior Director of Finance and then Acting Chief Financial Officer at ATS Medical before becoming Chief Financial Officer in 2007. Prior to joining ATS Medical, he was an independent financial consultant to the company. His career includes Controller at CABG Medical, Inc., a cardiovascular device manufacturer, and Corporate Finance Manager at Ecolab, Inc., a developer and marketer of products and services to the hospital, foodservice, healthcare and industrial markets. He also was a manager in the assurance and advisory services practice at the professional services firm of Ernst & Young LLP.

Steven Sobieski. Mr. Sobieski has served on our board of directors since 2006. Mr. Sobieski currently serves as chief financial officer and vice president of finance and administration of LifeCell Corporation, a position he has held since 2000. Prior to joining LifeCell Corporation, Mr. Sobieski was vice president of finance at Osteotech, Inc. From 1981 through 1991, he served in various positions with Coopers & Lybrand, a public accounting firm. Mr. Sobieski earned a Bachelor of Science from Monmouth University and a Master of Business Administration from Rutgers University. He is a Certified Public Accountant.

David R. Elizondo joined ATS Medical in October 2006. As Vice President, Research and Development, he has broad and practical experience and exposure to the research, development and operational functions of cardiovascular device design and manufacturing; particularly with respect to mechanical and tissue based prosthetic heart valves. Before joining ATS Medical, he was the Director, New Business Development at Boston Scientific. He has also served as the Director, Bioprosthetic Devices for Cryolife, Inc. and held various positions and responsibilties at St. Jude Medical.

Mr. Janney is a Managing Director of Alta Partners focused on life sciences investments. Prior to joining Alta, he was a senior investment banker at Montgomery Securities where his responsibilities included public offerings, strategic advisory assignments and advice on mergers, acquisitions and divestitures for life sciences companies. Dan is currently on the board of directors of a number of public companies, including Corgentech, Inc. and Dynavax Technologies, Inc., where he is a founder and Chairman of the Board, and several private companies. In addition, he led Alta's investments in Endonetics, Ilex Oncology, Definity, Intermune and LJL Biosystems. Dan is also a member of the advisory board for the Rebecca and John Moores Cancer Center at University of California, San Diego. He holds a Bachelor of Arts in history from Georgetown University and a Master of Business Administration from the Anderson School at the University of California, Los Angeles.

Mr. Nohra is a co-founder of Alta Partners, and was also a partner at Burr, Egan, Deleage & Co., which he joined in 1989. He has been involved in the funding and development of notable medical technology and life science companies including Cutera, Innerdyne, R2 Technology, deCODE genetics and Vesica. ??Previously, Mr. Nohra was Product Manager of Medical Products with Security Pacific Trading Corporation. He was responsible for a multi-million dollar product line and traveled extensively in Korea, Taiwan, Hong Kong, China and Southeast Asia. ??Currently, Mr. Nohra serves on the board of directors of several private companies including AcelRx, ATS Medical, Carbylan, Coapt, Paracor, PneumRx and Vertiflex and is the Chairman of the Board of USGI Medical. He also serves on the board of directors of the Medical Device Manufacturing Association. ??He holds a Master of Business Administration from the University of Chicago and Bachelor of Arts in History from Stanford University.

Garrett Gruener is a co-founder of AltaPartners, and was also a Partner at Burr, Egan, Deleage & Co., which he joined in 1992. Garrett has more than two decades of experience in the fields of software development, systems engineering and corporate development. In 1982, he founded Virtual Microsystems, a successful communications software company that was later merged with a larger corporation. Garrett also founded Ask Jeeves, now Ask.Com, a leading Internet search engine which is now part of IAC. Garrett specializes in information technology and is on the board of directors of Nanomix. Garrett earned a Masters Degree from the University of California, Berkeley and a Bachelor of Science from the University of California, San Diego, both in Political Science with a focus on technology policy

Anthony B. Evnin, Ph.D. , a director since October 2006, is a General Partner of Venrock, a venture capital firm, where he has been a Partner since 1975. He is currently a member of the Board of Directors of Icagen, Inc., Infinity Pharmaceuticals, Inc and Sunesis Pharmaceuticals, Inc., as well as being on the Board of Directors of a number of private companies. Dr. Evnin received an A.B. in Chemistry from Princeton University and a Ph.D. in Chemistry from the Massachusetts Institute of Technology.

Charles T. Liamos joined MedVenture Associates in September 2006 as Executive in Residence. Charlie has more than 10 years' experience in finance and operations' leadership positions and 11 plus years in the diabetes market, developing technologies and growing businesses. Prior to joining MVA, Charlie was President and CEO of Fovioptics, Inc., a medical device company that focused on blood glucose monitoring. Before that, he served as Chief Operating Officer and Chief Financial Officer of TheraSense, Inc., and served on its Board of Directors. He joined TheraSense when it was a private company and was a key person involved with the company's successful IPO and later the sale of TheraSense to Abbott Laboratories. When Abbott acquired TheraSense in 2004, Mr. Liamos was named Group Vice President of Business Operations for Abbott Diabetes Care, and served on the committee that integrated TheraSense into its new parent company. He previously has worked in operations management at LifeScan, a division of Johnson & Johnson. He began his career at General Electric.

General Partner, Alta Partners [-] Edward Hurwitz is a General Partner at Alta Partners, which he joined in 2002. Prior to joining Alta, he was Senior Vice President and Chief Financial Officer of Affymetrix from 1997 to 2002. Prior to that, he served as a Biotechnology Research Analyst for Robertson Stephens & Company from 1994 to 1997 and Smith Barney Shearson from 1992 to 1994. Mr. Hurwitz also practiced commercial law at Cooley Godward L.L.P. from 1990 to 1992. He serves on the Board of Directors of Avid Radiopharmaceuticals, Calistoga Pharmaceuticals, Cara Therapeutics, FoldRx Pharmaceuticals, Sunesis and Taligen Therapeutics. Mr. Hurwitz received a Juris Doctor from the University of California at Berkeley's Boalt School of Law and earned an M.B.A. degree in 1990 from the Haas School of Business. Mr. Hurwitz also received a Bachelor of Arts in Molecular Biology from Cornell University in 1985.

Eric W. Sivertson, 55, has served as the President and Chief Executive Officer of Dymedix Corporation, which designs, manufactures and markets sensors to sleep labs worldwide, since December 2004. Prior to Dymedix, Mr. Sivertson was a partner in the Minneapolis office of DHR International Executive Search, which focuses on the medical device, diagnostic and healthcare supply industries, from 2003 to 2005. From 2002 to 2003, Mr. Sivertson was a partner at TMP/Highland Partners, an executive search firm. From 1999 to 2003, Mr. Sivertson served as President and Chief Executive Officer of netRegulus, Inc., a web-based regulatory and clinical information technology company. From 1997 to 1999, Mr. Sivertson served as President and Chief Executive Officer of Biocompatible Cardiovascular, Ltd. Preceding this, Mr. Sivertson served as Division President of International Cardiovascular and Cardiac Rhythm Management for St. Jude Medical, a medical device manufacturer and marketer, from 1985 to 1996. Mr. Sivertson started his career at American Hospital Supply Corporation and held various sales and marketing positions from 1976 to 1985, including as Vice President of Marketing for the Convertors Division. Mr. Sivertson was elected as a director of ATS Medical in January 2003. Mr. Sivertson also serves on the board of directors of Dymedix Corporation.

Gary Eichhorn has been a leader in the computer and medical industries for over thirty years, as well as an amateur musician. Gary is currently the Executive Director of the Music and Youth Initiative, a not for profit organization dedicated to providing underserved urban young people an opportunity to learn and perform music.(www.musicandyouth.org) as well as the President of Eichhorn Group, LLC, a technology consulting firm. Mr. Eichhorn serves on the Board of Directors of a number of innovative technology companies. Gary is a strategic advisor to the CEO's of several high growth companies. In addition, Mr. Eichhorn is an advisor to Investor Growth Capital. Gary Eichhorn was previously CEO of Open Market, Inc., a leading provider of Internet commerce software, where he was responsible for the vision of the company, including strategic corporate direction and growth. Joining Open Market in 1995, he led the company to its initial public offering in May 1996. During his tenure the company grew annual revenues from $1 million to over $80 million. Open Market pioneered new technologies for managing Internet transactions, for which the company received numerous patents, and created an industry-leading suite of software products for e-commerce and content management. Previously, as Group Vice President of Hewlett Packard's Medical Products Group, Mr. Eichhorn was responsible for more than 900 products and 5,000 employees worldwide. The Medical Products Group focused on patient monitoring systems, cardiac ultrasound systems, cardiographs and defibrillators. As Vice President of Hewlett Packard's Workstation Systems Group, he was instrumental in the successful development and marketing of the HP family of reduced instruction set "Risc" workstations, and co-led a consortium of HP, SUN, and IBM to develop standards for the Unix operating system. Prior to Hewlett Packard, Mr. Eichhorn held executive positions at Digital Equipment Corporation where he managed the development of laboratory and medical hardware and software, and multiprocessor servers based on the Intel architecture. Mr. Eichhorn is a 1975 graduate of Colgate University, where he earned a bachelor's degree in Chemistry. He also attended the Advanced Management Program at Harvard Business School in 1988.

Jonathan J. Fleming is one of our co-founders and is Chairman of our Board of Directors. Mr. Fleming served as Chairman from January 1998 to May 2005 and assumed the position again in October 2006. Mr. Fleming has served as the Managing Partner of Oxford Bioscience Partners, a venture capital firm specializing in life science technology since 1996. Prior to joining Oxford Bioscience, he served as a Founding Partner of MVP Ventures in Boston, Massachusetts. Mr. Fleming serves as a member of the board of directors of Imcor Pharmaceutical Co. (formerly Photogen Technologies, Inc.), a specialty pharmaceutical company focused on developing medical imaging pharmaceutical products. He also serves as Chairman of the board of directors of BioProcessors Corporation, a privately-held corporation, and as a director of seven other privately-held companies, including Leerink Swann & Company, Inc., a Boston-based investment bank specializing in health care companies. Mr. Fleming is a Trustee of the Museum of Science in Boston and is a Senior Lecturer at Massachusetts Institute of Technology's Sloan School of Management. Mr. Fleming received a B.A. from the University of California, Berkeley and a Masters in Public Administration from Princeton University.

Michael E. Meyers, M.P.H. has served as one of our directors since March 2002. Mr. Meyers has served as a Managing Partner of Trivium Capital Management LLC, a money management firm in New York since May 2002. From 2000 to 2003, Mr. Meyers served as a Managing Director and Partner of Global Biomedical Partners, a life sciences venture capital firm. From 1997 to 2000, Mr. Meyers served as Director, Biotechnology and Pharmaceutical Investment Banking at Merrill Lynch & Co. From 1993 to 1997, Mr. Meyers served as Vice President, Health Care Investment Banking at Cowen & Company. Mr. Meyers holds an A.B. in Biology from Brandeis University and a Master of Public Health in Health Policy and Management with Highest Honors from Columbia University.

Robert E. Munzenrider, 61, is a retired financial and operating executive. From 2000 to 2002, Mr. Munzenrider was President of Harmon AutoGlass, a subsidiary of Apogee Enterprises, Inc. In 1999, he served as Vice President and Chief Financial Officer of the Glass Services Segment of Apogee Enterprises. He also served as Executive Vice President and Chief Financial Officer of Eliance Corp., an e-commerce service provider, during part of 1999. From 1997 to 1998, Mr. Munzenrider served as Vice President and Chief Financial Officer of St. Jude Medical, Inc. From 1983 to 1997, he served as Vice President and Chief Financial Officer of three subsidiaries of Viad Corp. and predecessor companies, including Travelers Express Company, Inc., Restaura, Inc. and Bell Atlantic Systems Leasing, Int. (previously Greyhound Computer Corporation). Mr. Munzenrider is a Certified Public Accountant. Mr. Munzenrider also serves as a director on the boards of Viad Corp and CABG Medical, Inc., and is a Trustee on the University of Montana Foundation. Mr. Munzenrider was elected as a director of ATS Medical in June 2003.
