Dr. Hazen received his medical degree from Kirksville College of Osteopathic Medicine and completed an Ophthalmology Residency at Saginaw Osteopathic Hospital and Metropolitan General Hospital in Pinellas Park, Fla. He is a certified member of the American Academy of Ophthalmology, OCOO, AOA, , the Saginaw County Medical Society and Saginaw Osteopathic Society. He is an assistant clinical professor at Michigan State University and Ferris State University. Dr. Hazen is a member of the active medical staff at Covenant HealthCare, St. Mary's Medical Center and Health Source Hospital in Saginaw.
