Thomas M. Ryan, age 55, Chairman of CVS Caremark Corporation since November 2007 and President and Chief Executive Officer of CVS Caremark Corporation since May 1998; was Chairman of CVS Corporation from April 1999 until March 2007; also President and CEO of CVS Pharmacy, Inc. since 1994; director of Bank of America Corporation, and Yum! Brands, Inc.

David B. Rickard, 61, is Executive Vice President, Chief Financial Officer and Chief Administrative Officer of CVS Caremark Corporation, a retail pharmacy chain and provider of healthcare services and pharmacy benefits management. He has held this position since joining CVS in September 1999. Prior to joining CVS, he was Senior Vice President and Chief Financial Officer of RJR Nabisco Holdings Corporation from March 1997 to August 1999. Previously, he was Executive Vice President of International Distillers and Vintners Americas. Mr. Rickard has been a member of our Board since October 2001 and is Chairperson of the Audit Committee and a member of the Finance Committee. Mr. Rickard is also a director of Jones Lang LaSalle Incorporated.

Douglas A. Sgarro, age 49, Executive Vice President and Chief Legal Officer of CVS Caremark Corporation and CVS Pharmacy, Inc. since March 2004 and President of CVS Realty Co., a real estate develpment company and a division of CVS Pharmacy, Inc. since October 1999; Senior Vice President and Chief Legal Officer of CVS Corporation and CVS Pharmacy, Inc. from September 1997 to March 2004. Mr. Sgarro is a graduate of Hamilton College and the University of Virginia Law School. He is a director of the United States Chamber of Commerce.

Jonathan C. Roberts, 51, Executive Vice President, Rx Purchasing, Pricing and Network Relations, CVS Caremark Corporation since January 2009; Senior Vice President and Chief Information Officer of CVS Caremark Corporation from January 2006 until January 2009; Senior Vice President - Store Operations of CVS Pharmacy, Inc. from August 2002 until December 2005; Area Vice President of Stores from April 1997 until August 2002.

Troyen A. Brennan, M.D., age 53, Executive Vice President and Chief Medical Officer of CVS Caremark Corporation since November 2008. Previously, Dr. Brennan served as Executive Vice President and Chief Medical Officer of Aetna, Inc. from 2006 through 2008; from 1997 through 2006 he was President and Chief Executive Officer of Brigham and Women’s Physician Hospital Organization, and from 2000 through 2006 he was also President and Chief Executive Officer of Brigham and Women’s Physicians Organization. Dr. Brennan has his MD and MPH from Yale Medical School, a JD from Yale Law School and a Masters Degree from Oxford University, where he was a Rhodes Scholar.

Howard A. McLure, age 50, is President of Caremark Pharmacy Services. Mr. McLure was Senior Executive Vice President and Chief Operating Officer of Caremark Rx, Inc. from June 2005 until the closing of the CVS-Caremark merger, when he became President of Caremark Rx, L.L.C. Previously, he served as Executive Vice President and Chief Financial Officer of Caremark from May 2000 until June 2005. Mr. McLure joined Caremark as Senior Vice President and Chief Accounting Officer in June 1998. Mr. McLure is a Certified Public Accountant and a member of the American Institute of Certified Public Accountants.

Larry J. Merlo, age 52, Executive Vice President of CVS Caremark Corporation and President of CVS/pharmacy – Retail since January 22, 2007; Executive Vice President—Stores of CVS Corporation from April 2000 to January 2007; and Executive Vice President—Stores of CVS Pharmacy, Inc. since March 1998 and Executive Vice President—Stores of CVS Pharmacy, Inc. from March 1998 to January 2007.

Helena B. Foulkes, age 43, is the Executive Vice President and Chief Marketing Officer, CVS Caremark Corporation, a position she has held since January 2009. Previously, Ms. Foulkes was Senior Vice President of Health Services of CVS Pharmacy, Inc., from October 2007 through January 2009, Senior Vice President, Marketing and Operations Services from January 2007 through October 2007, and Senior Vice President, Advertising and Marketing from April 2002 to January 2007. In her fifteen-plus years with the Company, Ms. Foulkes has held positions in Marketing and Operations Services, Strategic Planning, Visual Merchandising and Category Management. She is a graduate of Harvard College and received an M.B.A. from Harvard Business School.

V. Michael Ferdinandi, age 57, Senior Vice President—Human Resources and Corporate Communications of CVS Caremark Corporation and CVS Pharmacy, Inc. since April 2002; Vice President—Human Resources, Organizational Development of CVS Pharmacy Inc. from April 1999 to April 2002.

Jonathan also developed his interest in video in high school. He joined CVS in 1987 after completing his work at Capital College in the field of telecommunications engineering. Over the years, Jonathan has pursued several areas of specialty within the company. He has staffed the repair shop as a bench technician, has participated in several large system installations, and presently is designing and engineering broadcast and performance venue systems. In addition to his accomplishments at Contract Video Specialists, Inc., Jonathan has also served as staff engineer for NBC News Source in Washington D.C.

Terrence Murray has served as a director of ChoicePoint since May 2002. He served as Chairman of the Board of FleetBoston Financial Corporation, a diversified financial services company, from 2001 to 2002 and served as Chairman, President and Chief Executive Officer from 1982 through 2001, except in 1988, when he served only as President and from 2000 to 2001, when he served as Chairman and Chief Executive Officer. He serves as a director of FleetBoston Financial Corporation, A. T. Cross Company, a producer of writing instruments, CVS Corporation, a retail drugstore chain, and Air Products and Chemicals, Inc., a gas and chemicals company.

Director since March 2007 Age 56 Mr. Brown has been Chairman of Broad and Cassel, a Florida law firm, since March 2000. From 1989 until March 2000, he was Managing Partner of the Orlando office of the firm. He is also a director of Rayonier, Inc., a real estate development, timberland management and cellulose production company. Mr. Brown previously served on the board of Caremark Rx, Inc. from March 2001 until the closing of the CVS/Caremark merger, when he became a director of CVS Caremark Corporation.

C.A. Lance Piccolo has been a member of NovaMed's Board of Directors since 2000. Mr. Piccolo was Chairman and Chief Executive Officer of Caremark International, Inc. from 1992 through 1996, following its spin-off from Baxter International, Inc. Mr. Piccolo continues to serve as Vice Chairman of Caremark Rx, Inc. (NYSE: CMX), a leading prescription benefits management firm. Over Mr. Piccolo's distinguished 25-year career at Baxter, he progressed through a series of sales, operating and executive positions, including Executive Vice President for over five years. He was responsible for Baxter's alternate site operations and oversaw its development into one of the industry leaders. He was also a member of Baxter's Board of Directors from 1988 through 1992. Since 1996, Mr. Piccolo has served as President and Chief Executive Officer of HealthPic Consultants, Inc., a strategic health care consulting firm. Additionally, he serves on the Board of Directors of Crompton Corporation (NYSE: CK), Physician Dynamics, Inc., Benchmark Medical, Inc., Wind Point Partners, National Nephrology Associates, Inc., MedAssets Advisory Board and Lake Forest Hospital Foundation. He is also a member of the board of trustees of the University of Chicago Hospitals and the Advisory Board of Northwestern University, Kellogg Graduate School of Management.

Director since March 2006 Age 54 Mr. Dorman is a Managing Director and Senior Advisor with Warburg Pincus LLC, a global private equity firm. From November 2005 until January 2006, Mr. Dorman served as President and a director of AT&T Inc., a telecommunications company (formerly known as SBC Communications). From November 2002 until November 2005, Mr. Dorman was Chairman of the Board and Chief Executive Officer of AT&T Corporation. Mr. Dorman is also a director of Motorola, Inc., a communications products company, and Yum! Brands, Inc., a quick service restaurant company.

Jean-Pierre (JP) Millon is co-founder of BLS, LLC, a consulting and investing entity based in Indianapolis. He is also a partner in Areks, a Swiss-based pharmaceutical service company. Until September 2000, he was President and Chief Executive Officer of PCS Health Systems, one of the country's largest pharmacy benefit managers. Prior to joining PCS, Mr. Millon was an executive with Eli Lilly and Co., PCS' former parent company. His career with Lilly spanned two decades and was highlighted by leadership positions in the United States, the Orient, Europe, and the Caribbean Basin. Most recently, Mr. Millon served as President and General Manager of Lilly Japan, K.K. and Vice President of the Lilly pharmaceutical division in Kobe, Japan. Mr. Millon is the non-executive Chairman of MPV, Inc., a private health services company, and is an advisory board member with Care Capital LLC, a health care venture fund based in Princeton, New Jersey. Mr Millon also serves on the Boards of AdvancePCS, and IphysicianNet. He is also a member of the Global Advisory Council of The American Graduate School of International Management; Northwestern University's Graduate School of Management Alumni Advisory Board; the Dean's Council of 100 of the College of Business at Arizona State University; and the Barrow Neurological Foundation Board of Trustees.
