Became our chairman of the board and chief executive officer in October 1999. From June 1997 until he joined us, Mr. Thiry was chairman of the board and chief executive officer of Vivra Holdings, Inc., which was formed to operate the non-dialysis business of Vivra Incorporated, or Vivra, after Gambro AB acquired the dialysis services business of Vivra in June 1997. From September 1992 to June 1997, Mr. Thiry was the president and chief executive officer of Vivra, a provider of renal dialysis and other healthcare services. From April 1992 to August 1992, Mr. Thiry was president and co-chief executive officer of Vivra, and from September 1991 to March 1992, he was president and chief operating officer of Vivra. From 1983 to 1991, Mr. Thiry was associated with Bain & Company, first as a consultant, and then as vice president. Mr. Thiry is also a director of Varian Medical Systems, Inc.

Allen R. Nissenson, M.D., FACP, became our chief medical officer in August 2008. He is an emeritus professor of medicine at the David Geffen School of Medicine at UCLA, where he served as director of the dialysis program from 1977 to 2008 and associate dean from 2005 to 2008. Dr. Nissenson was the president of the Southern California End-Stage Renal Disease Network from 2005 to 2007. Dr. Nissenson was the president of the National Anemia Action Council from 2001 to 2007. Dr. Nissenson was the president of the Renal Physicians Association from 1999 to 2001.

David T. Shapiro became our chief compliance officer and senior vice president in October 2008, having joined us in March 2008 as the deputy chief compliance officer. Prior to joining us, Mr. Shapiro was counsel at the Pepper Hamilton law firm from March 2007 through February 2008, during which time he represented health care clients in government investigations and compliance issues. From October 2003 through March 2007, Mr. Shapiro served as a trial attorney with the Civil Frauds Section of the United States Department of Justice. From June 1999 through October 2003, Mr. Shapiro was an attorney with the law firm Mintz, Levin, Cohn, Ferris, Glovsky and Popeo, P.C. in Washington, DC.

Dennis Kogod became our chief operating officer in January 2009 and prior to that, he served as our president-west beginning in October 2005. Laura Mildenberger became our chief people officer in July 2008, having joined us in October 2001 as vice president of operations. Prior to joining us, Ms. Mildenberger served as vice president of operations for the western U.S. for Matrix Rehabilitation from March 2000 to October 2001. From 1993 to 2000, Ms. Mildenberger served as a general manager for NovaCare Outpatient Rehabilitation. From 1988 to 1993, Ms. Mildenberger was the executive vice president/principal of Worker Rehabilitation Services, a multi-site physical rehabilitation company. Ms. Mildenberger began her career as an occupational therapist at the Mayo Clinic.

Mr. Whitney is a healthcare executive with broad experience in finance, corporate strategy, business development, mergers and acquisitions, and international business. He previously served as the Chief Financial Officer of DaVita, Inc., a $2 Billion publicly traded healthcare services company. Prior to this Mr. Whitney served in various executive positions including, Director Corporate Development, Vice President Corporate Development and Vice President International for Total Renal Care, Inc. Mr. Whitney was also associated with RFE Investment Partners, a private equity investment firm and began his career at Touche Ross & Co. (now Deloitte & Touche). Mr. Whitney currently serves as a director of Light Sciences Corporation. Mr. Whitney, a CPA, earned a BS degree in Business Administration from the Schreyer Honors College of the Pennsylvania State University and a MBA from the Harvard Business School.

Georgina Randolph became our senior vice president in December 2001 and prior to that, she served as our vice president beginning in October 1993. From June 1989 until joining us, Ms. Randolph served as nephrology program director for the University of California, San Diego Medical Center. From 1988 to 1989, Ms. Randolph served as the director of north east operations for Renal Care Centers Corporation, which was acquired by Fresenius Medical Care Corporation in 1989.
Javier Rodriguez became our senior vice president in 2006. From 2004 to 2006, Mr. Rodriguez served as our vice president of operations. From 2000 to 2003, Mr. Rodriguez served as our vice president of payor contracting. From 1998 to 2000, Mr. Rodriguez served with us in various other director positions.

In the public sector, he has been part of the Executive Teams of CIGNA, 3M, HealthAmerica, and Vivra Inc. James K. Hilger became our vice president and controller in May 2006 and prior to that, he served as our vice president, finance beginning in September 2005. Mr. Hilger was our acting chief financial officer from November 2007 through February 2008. From September 2003 until joining us, Mr. Hilger served as vice president, finance and administration and chief financial officer of Pyramid Breweries, a brewer of specialty beverages. From December 1998 to July 2003, Mr. Hilger served in positions as chief executive officer and chief financial officer of WorldCatch, Inc., a seafood industry company. From 1987 until joining WorldCatch, Inc., Mr. Hilger held a variety of senior financial positions in the food industry. Mr. Hilger began his career in public accounting with Ernst & Whinney.

Mary Kay Hensley, MS, RD, CSR has provided renal nutrition therapy since 1968, first in Nebraska and then in Indiana. She currently is employed by Davita, Inc. in Gary, Indiana. A long time volunteer of the National Kidney Foundation, she has served as the CRN Chairperson and has filled many other positions within National CRN. She was instrumental in starting the Journal of Renal Nutrition during her tenure and served on its first editorial board. She serves on the Board of Directors of the National Kidney Foundation of Indiana and is a past-President. She is an active member of the International Congress of Renal Nutrition and Metabolism and has traveled to many countries to educate about renal nutrition and dietetic practice. Mary Kay has served on the ADA Specialty Practice Committee and has been a member of the work group to develop the Renal Specialty Certification exam. She just presented a paper on African cooking and the renal diet at the Spring Clinical Meeting and has a book chapter on the History of Renal Nutrition in press.

Mr. Vaughan was elected to the Board of Directors in August 2007. He served as Executive Vice President and Chief Financial Officer of Lincoln Financial Group from 1995 until his retirement in May 2005. He joined Lincoln in July 1990 as Senior Vice President and Chief Financial Officer of Lincoln National s Employee Benefits Division. In June 1992, he was appointed Chief Financial Officer for the corporation. He was promoted to Executive Vice President in January 1995. He was previously employed with EQUICOR from September 1988 to July 1990, where he served as a Vice President in charge of public offerings and insurance accounting. Prior to that, Mr. Vaughan was a Partner at KPMG Peat Marwick in St. Louis, from July 1980 to September 1988. Mr. Vaughan is a member of the Board of Directors of DaVita Inc. Age 59.
Ray joined Cutlass Capital in 2001 as a member of our Advisory Board and as a Venture Partner in 2006. Ray has over 30 years of health care industry experience. Prior to Cutlass Capital, he was CEO of Eunoe, a venture-backed start-up medical device company, from 2003 to 2005. From 1983 to 1997, Ray served as CEO of Nellcor Puritan Bennett (NYSE: NPB), a $1 billion revenue medical device company with over 4000 employees sold to Mallinckrodt for $2 billion. He was also the National Sales Manager for Bentley Laboratories from 1976 to 1983 and served as a Captain in the United States Marine Corps from 1970 to 1975. Ray has also been a Principal of health care merchant bank Group Outcome since 2000 and has served as Chairman and Director on the board of numerous public and private health care companies. He is also a former President of AdvaMed (Advanced Medical Technology Association, previously Health Industry Manufacturers Association). Ray is the Chairman of the Board of Trustees of St. Mary's College (Moraga, CA) and has a BS from LaSalle University.

Paul Diaz is currently President and Chief Executive Officer of Kindred Healthcare, Inc., one of the largest providers of healthcare services in the United States. Kindred is a New York Stock Exchange listed company with revenues in excess of $4 billion and over 54,000 employees in 40 states.
Kindred operates a diverse blend of health care service businesses including long-term acute care hospitals, skilled nursing facilities and contract rehabilitation sites in more than 600 locations across the United States. Kindred Healthcare was ranked first among “Health Care: Medical Facilities” in Fortune magazine’s list of Most Admired Companies in March 2009.
Mr. Diaz is an attorney and accountant, earning a bachelor’s degree in Finance and Accounting from American University's Kogod School of Business and a law degree from Georgetown University.
Mr. Diaz currently serves on the Board of Directors of DaVita (NYSE: “DVA”), and the Board of Visitors of Georgetown University Law Center. He was formerly on the Board of PharMerica Corporation (NYSE:PMC), the Board of the Bloomberg School of Public Health at Johns Hopkins University, and the Board of Trustees and Executive Committee of the Suburban Hospital Healthcare Systems in Bethesda, Maryland.
In 2008, Modern Healthcare magazine named Mr. Diaz one of the 100 Most Powerful People in Healthcare and one of the top 25 Minority Executives in Healthcare. In addition, in both 2008 and 2009 Hispanic magazine named Mr. Diaz as one of the 25 Best Latinos in business.
