Eric Matayoshi is Chief of General Surgery at Kaiser Permanente in Hawaii. He graduated from JABSOM, completed his general surgery residency in the University of Hawaii Surgical Residency Program, and completed a fellowship in Burns at the New York Hospital/Cornell University. He is an active member of a number of medical organizations and has taken part in many medical missions to underserved countries.

Francis J. Crosson, MD, is the executive director of The Permanente Federation, the national umbrella organization for the regional Permanente Medical Groups, which comprise the physician component of Kaiser Permanente. He assumed the leadership of the Permanente Federation (TPMG) in 1997 following a career with The Permanente Medical Group of Northern California, which he joined in 1977. While at TPMG he practiced pediatrics at the Hayward Medical Center with a subspecialty in infectious diseases. Crosson went on to distinguish himself in a variety of leadership and administrative positions. His work as a Permanente physician has convinced him that being part of a prepaid, multispecialty group practice is the best way to practice medicine and he is passionately dedicated to increasing group practice in American medicine. The Permanente Federation Medical Groups employ more than 11,000 physicians who care for approximately 8.2 million Kaiser Permanente members. Crosson also serves as the CEO and president of The Permanente Company, which carries out the Federation's business functions. In addition, he serves as chairman of the board of the Council for Accountable Physician Practices, and on the California Medical Association Board of Trustees and the Council on Legislation. Crosson received an undergraduate degree in political science and in 1970, a medical degree from Georgetown University. He completed a residency in pediatrics at the New England Medical Center Hospitals and a fellowship in infectious diseases at the Johns Hopkins University Medical School. He is certified by the American Board of Pediatrics. Prior to joining TPMG he served in the U.S. Navy for two years at Bethesda Naval Hospital.

Jill M. Steinbruegge, M.D., joins the faculty of the Harvard Medical School/Levinson Institute seminar Leadership for Physician Executives through an agreement with Kaiser Permanente. She is associate executive director for physician development for Kaiser Permanente and, in this role, is responsible for the design, implementation, management, and evaluation of all national programs for physician education and leadership development. Dr. Steinbruegge has held a variety of senior executive positions with Kaiser Permanente, including operations, staff, and consulting positions. She has been leading senior management teams and teaching leadership to physicians and other healthcare professionals for over ten years. Dr. Steinbruegge's particular areas of interest are physician leadership development, change leadership, executive coaching, medical group governance, developing effective teams, and organizational alignment to facilitate change. She teaches the leadership of change in the Advanced Leadership Program at the Kenan-Flagler School of Business at the University of North Carolina at Chapel Hill. Dr. Steinbruegge is a board certified psychiatrist. She received her M.D. from the University of Colorado, where she completed her internship in internal medicine and residency in psychiatry.

Dr. Barrett is Director of Research for the physicians of Kaiser Permanente of Colorado (KPCO). He has published several articles regarding patients' input in important healthcare decisions, including antibiotics for bronchitis, surgery for low back pain, and the completion of living wills. Dr. Barrett chairs the Guidelines Committee of KPCO and the national committees for Evidence-based Guidelines and Guidelines for Kaiser Permanente's electronic medical records.

As a licensed MFT, Jeri counsels individuals, couples and families on a variety of issues including depression, stress management, anxiety, eating disorders and weight management. In addition to her counseling practice, Jeri is a Project Coordinator/Manager for Kaiser Permanente, where she has developed and coordinates the Pediatric, Adolescent and Adult Obesity Programs, as well as the Bariatric Surgery Program, the Medical Weight Management Program and the Lifestyle & Weight Management Counseling Protocol. She has been involved in numerous health research projects including: Psychological Effects of Hospitalization for John Hopkins University, Stanford University's Diabetes Prevention Trial and she recently co-authored a four-year study entitled "Tobacco Cessation", focusing on the interrelationship of the psychological and physiological affects of tobacco cessation. Jeri has a Bachelor of Science degree in Human Genetics Biology/Chemistry from Arizona State University and a Masters Degree in Psychology and Counseling from University of San Francisco. She is past Board President of the Diabetes Society of Sonoma County and a member of the California Marriage & Family Therapists. She brings 18 years of experience to her work.

Wendy Lang, CEO of Spider Productions, is an outside director and IT consultant to Cambridge. She manages new technology for Cambridge as well as for Cambridge's clients. As a Web Producer and Web Marketer, her company, Spider Productions, places websites in the top 20 listings on the Search Engines. Previous to Spider Productions, from 1994 - 1999 she was the E-Business Web Portal Project Manager for Blue Shield. She managed the requirements for Charles Schwab's foreign exchange API. As a System Design Review Manager for Kaiser Permanente, she managed the reviews for Kaiser's National Clinical Information System, a $1.5 billion dollar IT project. As an IT Project Manager for Pacific Bell/Southwestern Bell, she Implemented new technology in competitive telecommunications products that include DSL, Wireless, Intranet and Internet technologies, Frame Relay, ATM, SONET, ISDN. She began her career in IT as a Vice President of CitiBank from 1986 - 1993 where she managed many projects including Home Computer Banking, and the global implementation of a $20 million dollar rollout of a system that supports all U.S., European and Asian customers. Other projects include: Development Director of Games for Girls, a multimedia startup; Multimedia Project Manager on a video documentary in Kathmandu, Nepal for the Asia Foundation and International Center for Self Governance; Art instructor and lecturer at Lafayette College, New York and Pennsylvania art organizations. She is a member of the Board of Directors of GraceNet - women in technology and Secession Gallery, San Francisco.

Mr. Folk graduated with "great distinction" from the Physical Therapy Program at California State University, Long Beach in 1987. He first became acquainted with Brian Mulligan in 1996 and has assisted Mr. Mulligan on courses since 1998. He received full accreditation as an instructor in the Mulligan Concept Teachers Association in 2001. He is a graduate of the Kaiser Permanente Orthopedic Manual Therapy Fellowship and currently serves as an adjunct instructor in the Fellowship program. Additionally, he is a Fellow of the American Academy of Orthopedic Manual Therapists. He has worked in outpatient orthopedic clinics since 1988 and currently works as an outpatient Clinical Specialist at Kaiser Permanente in San Marcos California. Dr. Williams graduated from the University of Utah School of Medicine prior to his successful residency at the University of Michigan in Ann Arbor. Since his residency, he has been working as a dermatologist with Kaiser Permanente near Washington D.C. Dr. Williams opened a practice in the Salt Lake Valley in November. Having grown up in Holladay, he is excited about this new opportunity to return home and serve the great people in the Salt Lake Valley. Carla joined Kaiser Permanente in July 1977, and currently works as the Assistant to the Vice President of Health Plan Services. She joined KaiPerm in 1982, and has served on the Board of Directors since May 1990. Of her choice to serve, she says, "I decided to run for a position on the Board of Directors because I felt I could bring some administrative and organizational experience to the group. I have found over the years that I have learned much more than I have imparted. I am passionate about the principle of members helping/serving each other. I have enjoyed watching the institution mature, grow, and develop into a stronger organization."

Cheryl is a Family Nurse Practitioner licensed in North Carolina and is a member of the Council of Nurse Practitioners and the American Nurses Association. She received her Masters of Nursing degree at University of North Carolina, Chapel Hill. Cheryl is a native North Carolinian. She joined Family Medical Associates of Raleigh in 2002 after working for Kaiser Permanente and in private practice. Cheryl enjoys spending time with her family, gardening and cooking.

Dr. Young has been working in women's health since 1981 having completed residency in Obstetrics and Gynecology at Kaiser Permanente, Oakland in 1985. She attended medical school at the University of California at Davis. She has practiced medicine around the world and the United States and has had a private practice in Logan, UT since 1989. She has been nationally recognized for her work in endometrial ablation which is a permanent treatment for abnormal bleeding. (see FYI). She is the physician resource ("ASK DR. YOUNG") for a national web site aboutheavyperiods.com which has received a first place award from the 2006 Insight Award for Customer Advocacy in Healthcare Communications. She has presented results of her patient care at national meetings for "The World Congress on Minimally Invasive Gynecology" and the "American Institute of Ultrasonic Medicine". Because of a practice mission to reduce unnecessary hysterectomy, she travels around the country to share practice outcomes with her peers.

Dr. David Cornish is the Chief of Adult Primary Care Medicine. He has been with Kaiser Permanente since 1985. His office is located on the 3rd Floor of the main building. He attended medical school at Boston University School of Medicine, Boston, MA. After graduating, Dr. David Cornish completed his internship and residency at Children's Hospital in San Francisco, CA and his fellowship at the University of California, Davis Medical Center, and Sacramento, CA. When not practicing medicine, Dr. David Cornish enjoys coin collecting, space science, history, and collecting original documents. His philosophy of care which he believes in is that the true mission of the physician was best expressed by Ambrose Pare which was to cure occasionally, to relieve often, and to comfort always. Julian Shelbourne is the Lead Metadata Architect for the deployment of the distributed Enterprise Metadata Repository at Kaiser Permanente. Prior to this, he was the Lead National Architect for meta-data repository and data-warehouse deployment projects at Computer Associates - working on many projects at Fortune 500 companies accross North America over a period of 6 years. Julian brings a hands-on understanding of meta-data modelling, coupled with a best practice approach to ensure alignment between the users and the developed solution.

Leslie received her Bachelor's degree in psychology from the University of California at Santa Barbara in 1978 and a Certificate in Physical Therapy from the University of California, San Francisco Medical Center in 1983. She graduated from the Kaiser Hayward PT Residency in 1990. She is a member of the American Academy of Orthopedic Manual Physical Therapists. She has many years of experience in orthopedic physical therapy at private practices and hospitals in San Francisco and in the East Bay. In addition to teaching for Kaiser Hayward, Leslie is currently a senior physical therapist at Kaiser Permanente in Oakland

Lynn Garske is Kaiser Permanente's Manager of Environmental Stewardship. She is responsible for directing environmental stewardship efforts to ensure Kaiser Permanente's business practices are aligned with protecting the environment by planning, organizing, and driving environmentally conscious purchasing practices, facilities design, and business operations. Additional focuses for Environmental Stewardship in 2005 include work on economically feasible, ecologically healthy food practices and implementation of a far-reaching chemicals policy promoting producer responsibility and positive health outcomes. Lynn has been with Kaiser Permanente for nine years.

Mandy Weltman received her Master's in Counseling from Sonoma State University in 2000, where her training focused on family, child, and adolescent therapy. She has been licensed as a Marriage and Family Therapist in California since 2002, and has worked in elementary, middle, high schools and community agencies. Ms. Weltman joined North Bay Behavioral Health (NBBH) in 2004. She works with children, adolescents, and their families to provide support in an open, flexible, and non-judgmental environment. Rather than subscribing to one particular mode of therapy, Mandy considers each client's individual needs and adapts her treatment approach accordingly. When not at NBBH, Mandy works at Kaiser Permanente as a Pediatric Clinical Health Educator. At Kaiser she work with pediatric patients and their families to encourage and support positive lifestyle changes. Her area of focus includes, but is not limited to, mental health, adolescent at-risk behavior, weight management issues, and parenting.

Mark is a physician with Kaiser Permanente in the East Bay. He also serves as an assistant clinical professor at UCSF Medical School. In addition, he volunteers at San Francisco General Hospital, where he supervises medical students and residents. Mark attended the Medical College of Wisconsin and did his residency at the University of Texas Health Science Center in Houston. He joined the board in 2007.

Mary Cranley works as a nurse practitioner at Kaiser Permanente in Hayward and is a member of the San Francisco Bay Area Physicians for Social Responsibility Food Working Group. She offers Reiki at a women's cancer clinic in Oakland and is also involved in a food security group working with issues surrounding genetically engineered food and agriculture.

Mitsuo has been a family practice physician at Kaiser Permanente since 1975. He serves on the faculty at the UCSD School of Medicine in the Department of Family and Preventive Medicine as an Assistant Clinical Professor, and chairs the San Diego Mayor's Asian/Pacific Islander Advisory Board. He attended UCLA for his undergraduate degree, received his medical degree from the UCSF School of Medicine, and completed his internship and residency training at the UCSF-affiliated program in Fresno.

Dr. Patricia Fahy is the Associate Medical Director of Human Resources for the Colorado Permanente Medical Group (CPMG). Fahy joined CPMG in 1999 and oversees recruitment and selection of physicians, orientation programs, physician leadership training, performance management and reward and recognition. In addition Fahy is the chairman of the credentialing committee and functions as a liaison for the medical group to work with the labor unions that represent nursing and other staff members of Kaiser Permanente. Prior to her current role, Fahy practiced internal medicine at the Westminster Kaiser Permanente Clinic where she served as chief of internal medicine. Fahy also practiced with the Southern California Permanente Medical Group in San Diego, California from 1988 until 1990 and participated on the CPMG Board of Directors from 1996 to 1999. Fahy received an undergraduate degree in psychology and continued her education at the University of North Dakota School of Medicine where she received her medical degree. Fahy completed her Internal Medicine Residency at Good Samaritan Medical Center in Phoenix, Arizona.

Dr. Paul Docktor, M.D. has been an orthopedic surgeon at Kaiser Permanente for 18 years. He is currently on the Sports Medicine team but his other specialty is foot and ankle surgery. Paul graduated from Medical School in Belgium (he's a fluent French speaker and loves all things French.) He did his residency at Johns Hopkins and Michigan and a Sports Medicine fellowship in Ontario, Canada with Rich Hawkins of the famous Stedman Hawkins Clinic in Vail, Colorado.

Philip A. Marineau has served on our Board of Directors since February 2007 and currently is the chairman of the Board. Since October 2008, Mr. Marineau has been partner of LNK Partners, a private equity firm. From 1999 to 2006, Mr. Marineau served as the President and Chief Executive Officer of Levi Strauss, & Co. From 1997 to 1999, he served as President and Chief Executive Officer of Pepsi-Cola North America. He currently serves on the Board of Directors of Meredith Corporation and is the chair of Meredith Corporation's Audit Committee. Mr. Marineau holds a Bachelor of Arts degree from Georgetown University and a Master of Business Administration degree from Northwestern University. There are no family relationships among any of our directors and executive officers.
