Mary Jane Jamar is vice president of Human Resources Management at MSH. Sue J. Goldie is a professor in the Department of Health Policy and Management and director of the Program in Health Decision Science at the Harvard School of Public Health. Trained as a physician, decision scientist, and public health researcher, she is best known for bringing together a wide variety of disciplinary approaches to address critical global health challenges. Asociacion Pro-Bienestar de la Familia (APROFAM) provides a wide variety of services related to family planning (FP), reproductive health (RH), maternal and child health, laboratory testing, and other basic health programs throughout Guatemala. APROFAM's target population is the lower- and lower-middle classes. Its urban program includes 32 clinics spread throughout the country and its Rural Development Program (RDP) serves some of the country's poorest, including the majority indigenous Mayan population in the highlands. With a 35-year history as the country's principal FP/RH agency, APROFAM has significantly improved Guatemala's access to family planning. APROFAM is the International Planned Parenthood Foundation (IPPF) affiliate in Guatemala. It has also received substantial financial support from the United States Agency for International Development (USAID).

Karen Hays, a dedicated certified nurse midwife working with JHPEIGO, left her home in Seattle to help train physicians and midwives as part of Management Sciences for Health's USAID-funded REACH program in Afghanistan. However, nothing could have prepared Hays for the experience awaiting her in Jalalabad, where she was on assignment working with International Medical Corp's (IMC) Midwifery Training School and supporting continuing education at Jalalabad's University Hospital. Sparked by the Newsweek Magazine report that soldiers had flushed a copy of the Koran down the toilet at Guantanamo Bay, enraged citizens in Jalalabad stormed the streets, targeting foreigners and international aid organization workers. Undeterred by danger and violence, Hays couragously assisted her patients and enlisted in the help of gracious Afghans who brought her to safety.

At the close of the 20th century, the world of philanthropy faces two very significant realities: (1) an imminent large-scale intergenerational transfer of wealth, and (2) a prolonged slump in charitable giving in the United States. In light of both factors, the role of the advisor in philanthropy is powerfully strategic. Karoff, president of The Philanthropic Initiative, argues that advisors to the wealthy have been far too narrow in perspective, usually giving counsel only in terms of effective tax planning. Karoff pleads instead for a "strategic philanthropy" that considers such crucial factors as the goal of empowerment, a posture of pro-activity, donor involvement and competence, and the significance of a donor's religious and moral values. He further suggests a "philanthropic curve" of donor development, characterized by increasing donor involvement in the giving process and by the alignment of the donor's values with his giving. Karoff issues a call for philanthropic advisors to frame the issue in this deeper and broader scope.
