Joshua H. Levine has served as President and Chief Executive Officer of Mentor Corporation since June 2004 and was appointed to Mentor Corporation s Board of Directors at that time. Mr. Levine began his career with Mentor Corporation in October of 1996 as Vice President, Sales-Aesthetic Products and advanced through positions of increasing responsibility in the aesthetic business franchise. In June of 2002, Mr. Levine was named Senior V.P., Global Sales and Marketing and an executive officer of Mentor Corporation. In December of 2003, Mr. Levine was promoted to President and Chief Operating Officer, the position he held until being named to his current position as Chief Executive Officer. Prior to joining Mentor, Mr. Levine was employed from 1989 through 1996 with Kinetic Concepts, Inc., a specialty medical equipment manufacturer, in a variety of executive level sales and marketing positions, ultimately serving as Vice President and General Manager of KCI s Home Care Division and a member of KCI s executive management committee. Mr. Levine began his career in healthcare with American Hospital Supply Corporation in 1982 and advanced over the next six years through a variety of sales and marketing management positions with that organization and its successor, Baxter Healthcare. Mr. Levine earned his bachelor s degree in Communications from the University of Arizona.

Edward S. Northup has served as Vice President and Chief Operating Officer since February 2007. Prior to joining us, Mr. Northup was employed with Boston Scientific Corporation for nine years and served most recently as President of Boston Scientific s pain management business. Mr. Northup joined Boston Scientific in 1997 as Vice President, General Manager of Asia Pacific. In 1999, he was promoted to President, Boston Scientific Japan and in 2001 to the concurrent role of President, Boston Scientific International. From 1995 to 1997, Mr. Northup was the President of the Dynacor Division of the privately-held Medline Industries. From 1978 to 1995, Mr. Northup was employed by Baxter Healthcare and American Hospital Supply Corporation in a variety of senior level positions and businesses, including Vice President of Baxter Cardiovascular-Far East, Vice President, General Manager of Euromedical Industries and Director of Operations for the Pharmaseal Division. Over the past 28 years, Mr. Northup has lived and managed businesses in North America, Latin America, Asia/Pacific and Europe. Mr. Northup earned his bachelor s of science degree in Pre-Med from the University of Santa Clara and began his career in basic research in intracellular immunity and infectious diseases at the Palo Alto Medical Research Foundation.

Michael O Neill has served as Vice President and Chief Financial Officer since November 2007. Prior to joining us, Mr. O Neill was employed with Johnson & Johnson, most recently serving as the Vice President of Finance, Worldwide Information Technology. From 2001 through 2007 Mr. O Neill was the Vice President of Finance, Chief Financial Officer for the Lifescan business, a $2+ billion leading supplier of blood glucose monitoring systems. Mr. O Neill joined Johnson & Johnson in 1987 with Site Microsurgical as a financial manager/analyst and moved through a progressively responsible series of positions including International Controller, Operations Controller, Finance Director and Group Finance Director before being named to the Chief Financial Officer position at Lifescan. Daniel Farkas has been a consultant to Mentor for over ten years, providing UNIX education to Mentor's clients. Dan has been involved in computing, technology and education for the past 30 years. He is currently the Chairman of the Information Systems Department at Pace University in Westchester, New York. His early experience was as a systems programmer on IBM mainframe computers in the financial industry gaining expertise in a spectrum of operating environments from the initial releases of OS/360 through MVS and VM. While staying current with the latest large systems technology, Dan began devoting his energy to smaller systems, becoming involved with the automation of foreign exchange operations and trading. Later, as a consultant, Dan was part of the original team which developed the technology plan for the new Chemical Bank, Delaware. After completing a MS degree in Computer Science from NYU's Courant Institute of Mathematical Sciences, Dan joined the Information Systems Department at Pace University. For almost 24 years he has been involved with the development and teaching of graduate and undergraduate curriculum. During this time he continued to work as a consultant to Fortune 500 companies primarily developing courses for mainframe application development. In 1984, Dan got involved with the UNIX operating system developing and delivering courses to professional audiences in all aspects of UNIX including C Programming. Neal McGrath, Ph.D. has been a well-known neuropsychologist in the Boston area for the last 25 years. He specializes in the evaluation and rehabilitation of traumatic brain injuries. Dr. McGrath is New England's first Credentialed ImPACT Consultant. His work has been featured on WCVB-TV's Chronicle and WGBH-TV's Greater Boston. Tracy Lee was born and raised in Sarasota, FL and is a graduate of Sarasota High School. He received a Bachelor of Science degree from the University of Florida in 1991 and a Juris Doctorate (J.D.) from Widener University School of Law in 1994. Mr. Lee was awarded an LL.M. (a degree awarded to law school graduates who complete an additional program of study) in Trial Advocacy from Temple University School of Law in 2008. Yvonne Johnstone joined Mentor in 2003 as an Employment Consultant in the South West. She has spent over 30 years in Human Resource Management roles in both the private and public sectors which included some roles in a Housing Association. She has experience in all aspects of training from training needs analysis, preparation of tailored training packages to delivery of individual courses. After gaining her B.A. at Exeter University she continued with her professional qualifications through the CIPD and later at the University of Portsmouth.

Rob Simon is the Senior Director of Global Business Development for the Automotive Forecasting group at J.D. Power and Associates. In this position, he is responsible for directing the international sales team and developing sales and marketing strategies for the automotive forecasting group as well as providing tactical direction for the analytical staff. Prior to joining J.D. Power and Associates, Rob was the Global Director of Marketing at Metaldyne. His responsibilities included business analysis, market assessment and strategic planning. Sheila Riggs was appointed the President and CEO of Delta Dental of Minnesota in July, 2005. Delta Dental of Minnesota is the largest provider of dental benefits in the Upper Midwest, serving 3.3 million individuals employed by more than 8,500 employers. The company has an A (Excellent) rating from A.M. Best and a AA- (Very Strong) rating from Standard & Poor's one of the top financial ratings for an oral health benefit company in North America. Riggs served on its Board of Directors from 1999 until her appointment as CEO. Prior to moving to Minnesota with her husband Scott, Riggs was an officer at Wellmark Blue Cross and Blue Shield of Iowa for 10 years where she held a succession of executive positions. In community and volunteer activities, her primary focus is health care and women's leadership issues. As Chair of Healthy Iowans 2010, Riggs led a group of 500 Iowans who created a blueprint for action to improve the health and well being of all Iowans by the year 2010. Dr. Riggs was the founding Dental Director of the Ames, IA Free Dental Clinic, a member of the Board of Directors of the United Way of Central Iowa, and an expert reviewer for the Food and Drug Administration. In 1999, the Governor appointed Sheila to serve on the Board of Directors of IPERS, the $17 billion retirement fund for public employees of Iowa. Dr. Riggs joined the Board of the YWCA of Minneapolis in 2006 and the Board of the Greater Twin Cities United Way in 2007. She is also President-Elect of the Women's Health Leadership Trust, a member of the Minnesota Women's Economic Roundtable, and a Mentor with Menttium 100. Riggs is frequently asked to speak to audiences of young women on the opportunities in the fields of science and math. She has two doctorates in health care: a DDS from the University of Iowa College of Dentistry in 1986 and a Doctorate of Medical Sciences in epidemiology from Harvard University in 1991. She maintains faculty positions at both her alma maters.
