Dr. Rosof is Senior Vice President for Corporate Relations and Health Affairs at the North Shore-Long Island Jewish Health System. Prior to assuming a full time Hospital position, Dr. Rosof was in the private practice of Internal Medicine and Gastroenterology. He is currently a Clinical Associate Professor of Medicine at The School of Medicine of the State University of New York at Stony Brook and an Attending Physician at Huntington Hospital, Huntington New York, where he is also a Vice Chair of the Board of Directors. Dr. Rosof is the Institutional Official for BRANY (Biomedical Research Alliance of New York), overseeing BRANY's Institutional Review Boards. Dr. Rosof has been a driving force in American Medicine. He has chaired Committees and Task Forces for the State of New York, The Institute of Medicine, The American Medical Association, and various Specialty Societies. He has achieved national recognition in the areas of health quality and clinical practice guidelines and is the current Chair of the Physician Consortium for Performance Measures convened by the AMA. Dr. Rosof has spoken nationally and internationally on issues of Quality and Patient Safety. Dr. Rosof is Past President of the American Society of Internal Medicine and The Internal Medicine Center to Advance Research and Education. He is Chair Emeritus of the Board of Regents of the American College of Physicians, and current Chair of the Foundation of the American College of Physicians.

Phil Gautreau has been Vice President for Network Development at the North Shore-Long Island Jewish Health System since November, 1998. The Health System is a network of 14 hospitals (2 of which have TBI units - Staten Island University Hospital and Southside Hospital in Suffolk County), 3 long-term care facilities, 5 home care agencies and numerous ambulatory care facilities providing care to patients in Long Island, Queens and Staten Island. Several of the System's hospitals also provide inpatient and outpatient rehabilitation services. Phil graduated in 1988 from Columbia's School of Public Health with a focus in Health Administration. His administrative residency at St. Vincent's Hospital and Medical Center in Manhattan led to a position as Program Director of the New York State Department of Health approved AIDS Treatment Center. He subsequently worked with Staten Island University Hospital for 8 years during which time he had responsibility for several departments including: Network Development, Regulatory Affairs, Quality Management, Case Management, Infection Control, Social Work, Employee Health, Employee Assistance Program, the Pediatrics Department and the AIDS Center program. Phil also lived in Moscow from Summer, 1994 - to Fall, 1995, and worked as the Business Director of a joint venture between Columbia-Presbyterian and Pepsi Cola International to develop and manage a medical office geared toward expatriates. A substantial portion of Phil's work in Network Development is related to developing relationships between the Health System's hospitals and community-based agencies to ensure continuity of patient care. Phil is a member of the American College of Healthcare Administrators, Metropolitan Healthcare Administrators (NYC), and the American Public Health Association. Phil is a member of the Community Advisory Board Member of Brain Injury Society since 1999 and a Special Advisor on Administrative Matters to the Executive President of Brain Injury Society.
Deborah M. Benson, Ph.D., is the Executive Director of Transitions of Long Island, a subsidiary of the North Shore-Long Island Jewish Health System in Manhasset, N.Y., specializing in brain injury rehabilitation. She is a New York State Licensed Clinical Neuropsychologist with over 10 years of experience in the field of neuro-rehabilitation. Dr. Benson earned her Ph.D. in Clinical Neuropsychology from the Graduate Center of the City University of New York in 1992, and joined Transitions in 1993. Prior to joining the team at Transitions, she was formerly with the Long Island Rehabilitation Center at Nassau in Great Neck, and the International Center for the Disabled in NYC. Her memberships include the American Psychological Association, National Academy of Neuropsychology, International Neuropsychological Society, and national and local chapters of the Brain Injury Association. She is certified by the American Academy for the Certification of Brain Injury Specialists as a Clinical Instructor and Clinical Examiner. She is also certified in Neurofeedback by the National Registry of Neurofeedback Providers, and by APA in the Treatment of Alcohol and Other Psychoactive Substance Use Disorders. She is listed in the National Register of Health Service Providers in Psychology. Dr. Benson has presented on numerous topics related to brain injury rehabilitation at both regional and national conferences.
Dr. Fastenberg earned his Bachelor of Arts from the State University of New York at Stony Brook and received his M.D. from New York Medical College. He completed an internship in Internal Medicine at Metropolitan Hospital Medical Center. His residency in Ophthalmology was completed at Northwestern University in Chicago. Dr Fastenberg subsequently completed his fellowship in Medical and Surgical Disorders of the Retina and Vitreous at the Estelle Doheny Eye Institute at the University of Southern California School of Medicine in Los Angeles. He has been a partner with Long Island and Queens Vitreo-Retinal Consultants (LIVR) since 1982.

Dr. Vivian Kafantaris is Associate Professor of Psychiatry and Behavioral Sciences at the Albert Einstein College of Medicine and Director of Research in Child and Adolescent Psychiatry for the North Shore-Long Island Jewish Health System. She completed medical school and a general psychiatry residency at the Albert Einstein College of Medicine. She went to NYU/ Bellevue for child and adolescent psychiatry training and fellowship training in pediatric psychopharmacology research under the direction of Dr. Magda Campbell. She is currently funded by the National Institute of Mental Health to conduct studies in the maintenance treatment of bipolar disorder in children and adolescents, ages 10 to 18 years.

Jean Elbaum, Ph.D. is the Clinical Director of Transitions . She has been with the program since it's inception in 1988. Dr. Elbaum holds a Ph.D. in Rehabilitation Counseling from New York University. She has many years experience in neuro-rehabilitation, beginning in 1980, when she completed an internship at NYU-Bellevue Hospital's Cognitive Rehabilitation Program. Prior to joining Transitions, Dr. Elbaum assisted in the development of specialized services for individuals with brain injury at United Cerebral Palsy of Nassau County. Dr. Elbaum's primary clinical interests include rehabilitation of memory and unawareness deficits, and individual, group and family psychotherapy to facilitate coping with adjustment to brain injury.

Robert A. Duarte, M.D. is a Board Certified Neurologist, who has been Transitions' Medical Director for the past 10 years. He is also Board Certified in Pain Management, and is a licensed Acupuncturist. Dr. Duarte has an appointment as Assistant Professor of Neurology at Albert Einstein College of Medicine. He is a member of numerous professional societies, including the American Pain Society, the International Association for the Study of Pain, the American Headache Society and the American Academy of Neurology.
Joanne Fulgieri, B.S. is Transitions' Operations Manager. She holds a Bachelor of Science Degree in Education from St. John's University, and has been with Transitions since 1990. Among her varied responsibilities, Joanne oversees the Admissions and Human Resource functions of Transitions' program.
Kevan Abrahams (D-Hempstead) is serving his first full term as a Nassau County Legislator in the 1st Legislative District, after decisively winning his seat by 84 percent of the vote in a special election in September 2002. The youngest legislator ever elected to the Nassau County Legislature at age 27, Abrahams comes to his new position with a wealth of experience gained from serving as the Majority's Finance Director prior to being elected. Abrahams is also the youngest African American lawmaker in the State of New York. Legislator Abrahams' priorities for his district, which is comprised of parts of Baldwin, Freeport, Hempstead, South Hempstead, Roosevelt, and Uniondale, include forming an economic community task force to address the need for business growth in many of the economically challenged areas and addressing the growing problem of gangs. Since taking office in 2002, Legislator Abrahams has been a strong advocate for the Counties poor, middle class, and working families. Abrahams has also put priority on enhancing senior services, after school programs, and the quality of life for residents of his district. Abrahams, a life-long resident of Hempstead, joined the County Legislature staff as Deputy Finance Director in 2000, eventually moving up to the position of Finance Director. He brings with him a wealth of government experience and has served as a legislative assistant and Deputy District Director for Congressman Floyd H. Flake and as a District Representative for Congresswoman Carolyn McCarthy. Abrahams also worked in the private sector as a Financial Analyst for Astoria Federal Savings Bank. He holds a Bachelors degree in Political Science from Queens College. Currently he works as an analyst for North Shore Long Island Jewish Health System in its Strategic Planning Department. In February 2004, Abrahams was featured in Ebony Magazine as one of its 30 under 30 leaders. This accolade is a testament to Abrahams' leadership abilities. He has also been recognized as the 2004 Citizen of the Year by the National Association of Social Workers among other achievements. Also during the 2003 Congressional Black Caucus, Abrahams was a panelist on the "Affirming the Playing Field Is Affirmative Action Necessary" panel. In addition, Legislator Abrahams has also appeared on several network programs to discuss gang prevention, medicaid, economic and job development, and education. Abrahams serves as chairperson of the Public Works Committee; vice chair of the Economic Development and Labor committee; and as a member of the Finance, Public Safety, Government Services and Minority Affairs committees. Legislator Abrahams is a tremendous sports fan and his favorite teams are the New York Knicks, New York Jets, and New York Yankees.

MICHAEL H. SAHN is the founding member of the Firm and concentrates his practice in the areas of zoning, land use planning, real estate law and transactions, corporate and environmental law. He also represents the Firm's clients in all facets of civil litigation. Mr. Sahn is a Trustee of the North Shore-Long Island Jewish Health System, Inc., Manhasset, New York, and a Member of Community Board of Southside Hospital in Bay Shore, New York. He is also Chairman of the Board of Zoning Appeals of the Incorporated Village of Flower Hill and serves on the Advisory Board of the Chicago Title Insurance Company. He is a Member of the Nassau County Bar Association and served as Chairman of the Administrative Agencies Committee of the Nassau County Bar Association from 1983 to 1985. Mr. Sahn is frequently invited to lecture before professional groups in his areas of expertise. Mr. Sahn began his legal career as a Deputy Town Attorney of the Town of North Hempstead, Nassau County, New York, in 1977. At the Town of North Hempstead, Mr. Sahn served as Counsel to the Board of Zoning and Appeals from 1978 through 1979, and as Senior Deputy Town Attorney in 1979. He then served as Commissioner of Planning and Research Development for the Town of North Hempstead from 1979 to 1981. In 1981, he joined the law firm of Margiotta & Ricigliano in Uniondale, New York, where he served as a partner of the firm. Additionally, from 1984 through 1997, Mr. Sahn served as Associate Counsel to New York State Senator Michael J. Tully, Jr. representing the Seventh Senate District in the New York State Senate. Mr. Lane is the Chief Executive Officer. Before joining Modern Bank, Mr. Lane was Chairman and Chief Executive Officer of Bear Stearns Asset Management. He was a Vice Chairman of Lehman Brothers, a member of the Office of the Chairman, Co-Chairman of Lehman Brothers Asset Management and Alternatives Division and Chairman and CEO of Neuberger Berman Inc. (NBI), as well as a member of NBI's Executive Management Committee. He joined Neuberger Berman, LLC in July of 1998 as a Principal and Chief Administrative Officer. Mr. Lane was previously with Travelers Group as Vice Chairman working closely with the Chairman and Chief Executive Officer in the management of the company's various administrative functions and operating units. He was responsible for human resources, corporate communications and investor relations functions. Before assuming those responsibilities in early 1996, he had been Vice Chairman of the company's Smith Barney subsidiary, responsible for asset management, mutual funds, international, futures and commodities division. Prior to his affiliation with Travelers Group, Mr. Lane spent his career, since 1969, with Shearson Lehman Brothers and its predecessor firms - certain businesses of which were combined with Smith Barney in 1993. From 1987-1989, he served as Shearson's President and Chief Operating Officer and was on the Board of Directors from 1982-1989. Prior to that he had been the firm's Chief Operating Officer, administrator of its capital markets division, and in various management positions in institutional sales, taxable fixed income trading and risk arbitrage. He began his career as a securities analyst in a predecessor firm to Shearson. Mr. Lane is a graduate of New York University and holds an MBA from the Columbia University Graduate School of Business. He is a Director of the North Shore/Long Island Jewish Health System, Chairman of the Finance Committee of the North Shore/Long Island Jewish Health System and a Director of the National Academy of Finance. He is also a Director of Willis Group Holdings. Born in 1942, he resides in New York City with his wife and they have five children. Michael Katz joined Sterling Equities in 1973 and currently serves as a Senior Executive Vice President. He is Co-Chief Executive Officer of the Sterling American Funds with day-to-day responsibility for the management of their investments as well as leading and directing Sterling Equities' and Sterling American Funds' in-house financing departments. Mr. Goldstein is a Senior Managing Director of Alpine Capital LLC and its affiliate, Alpine Equity Partners (collectively, "Alpine"). Alpine invests as principal in a variety of businesses, especially in the media, entertainment and telecommunications industries. In addition, it also provides financial advisory and merger and acquisition consulting services. Mr. Goldstein is also a Director and member of the Executive Committee of American Community Newspapers, Inc., an AMEX listed company. Prior thereto, Mr. Goldstein was a member of the law firm of Paul, Weiss, Rifkind, Wharton & Garrison, which he joined in 1976. Mr. Goldstein was a Director of The Berkshire Bank from 1991 to 1999, serving on the Board's Executive Committee and Loan Committee. Bob has been distinguished for his work and support for a significant number of charitable organizations. He is a major contributor and personally supports The Big Apple Circus Clown Care Unit, a community outreach program of the Big Apple Circus, currently operating at six New York hospitals including Schneider Children's Hospital of North Shore-Long Island Jewish Health System, as well as in Boston, New Haven, Seattle, Chicago and Washington D.C. Specially trained professional clowns visit hospitalized children and, while medical doctors treat their diseases, the "clowns" treat their funny bones and raise their spirits. He was honored by "1 in 9", Long Island's Breast Cancer Coalition, as well as by many of the organizations named herein. In addition, Bob is on the Executive Council of The Tilles Center Council of Overseers and will be the honoree at the 2003 Tilles Center for the Performing Arts Gala XXIII. In addition, Bob serves on the President's Council of the Gay Men's Health Crisis, is a Trustee of the North Shore/Long Island Jewish Health System, and is a co-chair of their Cancer Initiative as well as serving on the Advisory Board of Hofstra University's Elite Honors College. He is also a founding supporter of The Harlem Little League, and actively supports the Little League Baseball Urban Initiative. He donates generously to The Church of St. John the Baptist Food Pantry and supports The Walt Frazier Youth Foundation as well as Wynton Marsalis' "Jazz @ Lincoln Center" Program. He serves on the Board of the Diabetes Institutes Foundation, supports the Boys Club of New York, the Newark YM/YWCA, and numerous baseball, basketball and soccer teams throughout the metropolitan area. Most recently, Bob served as Chairman of the Nassau County Blue Ribbon Panel and sits on the Nassau Interim Finance Authority, to guide Nassau County's fiscal destiny. He is often called upon as an expert on urban, healthcare, and minority issues by many members of both State and Federal governments. Bob is also involved with H.E.L.P. (Housing Enterprise for the Less Privileged), which was founded by Andrew Cuomo in 1986 and is the nation's largest developer of housing and provider of services for the homeless. In addition, Bob is also a major supporter of the Neighborhood Housing Services of New York City. Additionally, in 1994 Bob received the Entrepreneur of the Year award in a competition sponsored by Inc. Magazine and Ernst and Young. In 1996, The Robert Plan was named Employer of the Year by Adults and Children with Learning and Developmental Disabilities, and in 1997 was honored by the National Center for Disabilities in Albertson. He was recently honored as the 1998 Corporation of the Year by The Long Island Hispanic Chamber of Commerce. In addition, the company developed the pilot for PROSPER, a scholastic internship program for "at risk" students on Long Island.

Steven R. Schlesinger is the managing partner of Jaspan Schlesinger Hoffman LLP and the partner in charge of the firm's Litigation Practice Group. The firm has over 60 attorneys in its New York and Delaware offices. Mr. Schlesinger concentrates his practice in commercial and real estate matters, bankruptcy, and litigation. He provides general advice and counsel to telecommunications, real estate development, automobile dealerships, and construction companies. He has also acted as special counsel to numerous government agencies and served as a director of a publicly held corporation. In addition, Mr. Schlesinger has extensive litigation experience involving Election Law. He currently serves as Law Chairman of the Nassau Democratic County committee and has served as a member of the Special Committee of the New York State Bar Association on the Reform of the Election Law. He as acted as counsel to numerous political campaigns including Clinton/Gore '96 and McCarthy for Congress. Mr. Schlesinger was appointed by the Appellate Division, Second Department, as a member of the Character and Fitness Committee and served as the Chairman of the Professional Ethics Committee of the Nassau County Bar Association. In addition, Mr. Schlesinger has represented numerous businesses in bankruptcies, commercial matters and litigations arising therefrom. Mr. Schlesinger has also had business interests involving real estate developments, car dealerships, automotive distributorships, oil reclamation systems, and restaurants amongst other businesses. Mr. Schlesinger received his undergraduate degree from Queens College and his Juris Doctor from Hofstra University School of Law. He is admitted to practice law in the State Courts of New York, and before the Supreme Court of the United States and the Eastern and Southern District Courts of New York.
