Kenneth C. Mitchell has served as Chief Financial Officer of Symbion since May 2002 and was named Senior Vice President in December 2002. Prior to assuming that position, Mr. Mitchell served as Chief Financial Officer of American HealthMark, Inc. from 1990 to 1996 and as Vice President-Controller for HCA Management Company from 1988 to 1990.

Prior to joining Symbion as Vice President of Acquisitions & Development, Mr. Bundren served as Vice President of Development with a competing ambulatory surgery center management company. He brings more than 25 years of financial management experience to the Company and has an extensive background in structuring partnerships and physician relationships. Previously, Mr. Bundren served as Chief Financial Officer and General Counsel of a telecommunications start-up company that was subsequently acquired. He also served as the Chief Financial Officer of Active Services Corporation, the largest provider of adult day care services in the United States, and a physician practice management company. Earlier in his career, he held a senior management position with a national accounting firm. A certified public accountant and licensed attorney, Mr. Bundren is a member of the American Institute of Certified Public Accountants and the American Bar Association.

Prior to joining Symbion as Vice President of Acquisitions & Development, Ms. Greene oversaw the acquisition and development of more than 250 facilities nationwide at National Surgical Care, United Surgical Partners International, and HealthSouth collectively. In addition to her extensive experience in the development of ambulatory surgery centers, Ms. Greene holds a Series 22 and Series 63 license as a Registered Representative and has successfully syndicated surgery center partnerships for more than a decade.

Robert V. Delaney joined Crestview as a partner in 2007. He retired from Goldman, Sachs & Co. in 2003 where he served in a variety of leadership positions including head of the Principal Investment Area in Asia, head of the Principal Investment Area in Japan and head of the Leveraged Finance Group. Mr. Delaney has extensive experience in high yield debt, leveraged loans, restructuring, mergers and acquisitions and principal investing. Mr. Delaney received an MBA from the Harvard Business School and he received a B.A. in Economics from Hamilton College.

Mr. Tyrrell has 20 years of venture capital experience as founding partner in three Richland Ventures funds and two Lawrence, Tyrrell, Ortale & Smith funds. Prior to entering the venture capital business, Mr. Tyrrell founded the Prudential-Bache Health Care Group, a specialized investment bank. Mr. Tyrrell also spent six years at Hospital Corporation of America as Vice-President of Finance. His responsibilities at HCA included capital market activity and acquisition analysis. Mr. Tyrrell has served as a director of numerous public and private companies including Oxford Health Plans, Symbion, Regal Cinemas, National Health Investors and Six Flags. He currently sits on five boards. Mr. Tyrrell received a B.S. degree in electrical engineering from the University of Tennessee and an M.B.A. degree from Harvard Business School.

Dr. Kurtin has over 20 years of experience as a healthcare executive and is a founding member of Pacific Venture Group (PVG). PVG was founded in 1995 and currently manages over $210 million in venture funds, investing in all aspects of healthcare including biotechnology, healthcare information technology, healthcare services and medical devices. Ms. Kurtin serves on the board of PVG companies Asteres, Centerre Healthcare Corporation, Familymeds, Vivant and Symbion (NASDAQ: SMBI) and is responsible for PVG's investment in Radiant Research. Dr. Kurtin was also responsible for PVG's investment in Zone Labs, recently sold to CheckPoint Software (NASDAQ: CHKP) and for Phase Forward (NASDAQ: PFWD) which completed a successful IPO in July 2004. In addition Dr. Kurtin is a founding member of the RAND Health Board of Advisors, and also serves on the Board of Advisors for the Harold Price Center for Entrepreneurial Studies at The Anderson School at UCLA, the President's Advisory Council for Barnard College, and the Board of Trustees for the American College of Physicians Foundation. Dr. Kurtin received a Doctorate of Pharmacy from University of Pacific and an MBA from the University of California, Los Angeles.

Kurt E. Bolin is a Principal of Stone Point Capital. Prior to the formation of Stone Point in 2005, Mr. Bolin was a Principal of MMC Capital. He joined MMC Capital in 1997 from GE Capital, where he held various private equity positions from 1986 to 1997, most recently as Managing Director of the Equity Capital Group. In addition to private equity investing, Mr. Bolin's activities at GE Capital included leading the acquisitions of various insurance companies. Mr. Bolin is a director of BLC Holdings, LLC, Wilton Re Holdings Limited and Mercator Risk Services, Inc. He holds a B.S. from Wake Forest University and an M.B.A. from the Wharton School of the University of Pennsylvania.

Co-founded Crestview in 2004. Retired from Goldman Sachs in 2003 where he was the co-founder (1994) and head of the Financial Sponsors Group. The Financial Sponsors Group was responsible for Goldman’s investment banking activities with private equity firms around the world. During Mr. Murphy's tenure, Goldman enjoyed market leadership with these clients in the firm's most profitable product areas. In addition to his investment banking activities, Mr. Murphy originated and/or managed several of Goldman’s direct private equity and mezzanine capital investments. Mr. Murphy is a director of Symbion, FBCM and Key Safety Systems. Mr. Murphy received his MBA from Harvard University, where he earned second year Honors. Mr. Murphy received his A.B. from Princeton University. While at Harvard, Mr. Murphy co-founded the Massachusetts Bay Brewing Company. He serves on the Boards of Trustees of The Greenwich Country Day School; The Inner-City Scholarship Fund; The Madison Square Boys and Girls Clubs; and The Taft School.
